Athletic Physicals
Students at Gage who want to participate in the after school sports programs need to complete an athletic physical packet. This packet includes a form that a doctor has to sign to clear them for sports.
You can pick up a copy of the packet at the Gage main office or download it from the link below. Just so you know, this is the same packet that high school students use.
Students have to turn in their completed packet and clearance form to the Gage office before they can try out for any team. Without the completed paperwork, students won't be able to participate in tryouts.
